Rita Grandelis, MSW, LCSW, CDP, CMC, has over 25 years of experience in healthcare and clinical social work, with a strong focus on adult and geriatric care. She earned her bachelor’s degree in psychology and sociology from the State University of New York at Cortland and her master’s degree in social work from Adelphi University. Rita is a New Jersey Licensed Clinical Social Worker (LCSW).
Rita holds multiple professional certifications that reflect her expertise in aging and care coordination, including Certified Dementia Practitioner (CDP) and Care Manager Certified (CMC). She is an active member of the Aging Life Care Association (ALCA).
Throughout her career, Rita has worked across a wide range of healthcare settings, including acute care hospitals, subacute and long-term care facilities, assisted living communities, and visiting nurse services. This diverse experience has provided her with a comprehensive knowledge base and a creative, solution-focused approach that benefits clients, families, and interdisciplinary care teams.
In May 2018, Rita joined Unicity Healthcare as Director of Geriatric Care Management. She was instrumental in developing and expanding Unicity’s Geriatric Care Management program. Under her leadership, services grew from serving northern New Jersey to multiple counties statewide, as well as extending into New York and Florida.
Rita’s extensive knowledge of healthcare systems, home care, senior living options, guardianship, and community resources is a valuable asset to Unicity’s clients, families, and staff. She is passionate about education and regularly provides educational programs and support groups for staff, families, professional colleagues, and senior living communities.
